The hallmarks of Tetralogy of Fallot

Tetralogy of Fallot is a form of a congenital heart defect that represents the combination of four heart defect symptoms that are present at birth. As a rare heart disease, it affects about 5 out of 10,000 children and it involves both genders equally. The classic physiological defect in the condition is the resulting abnormal blood flow through the heart owing to its structural defects. As a result, poor blood flow gets through the heart, depriving it with the proper nutrients and oxygen to carry out its normal function.

In Tetralogy of Fallot, not enough blood reaches the lungs in order to get sufficient oxygen to bring to the heart. As a result, poorly oxygenated blood reaches the heart. This is due to the four structural heart defects that are present in the disease that prevent the sufficient flow of oxygen throughout the body and in supplying the heart cells. These four structural defects of the heart are the hallmarks of Tetralogy of Fallot.

1.       Pulmonary stenosis

This is a condition where there is a narrowing of the pulmonary valve that prevents the normal flow of the blood to get some oxygen supply from the lungs. With a narrow passage on the right ventricle, the oxygen poor blood from the right ventricles are unable to pass through in order to reach the lungs to get some oxygen supply to the blood.

2.       Ventricular septal defect

The heart has a wall separating the two chambers located on either side of the heart which is called the septum. The septum plays an important role in preventing the mixture of the blood from both sides of the heart chambers.  The presence of a ventricular septal defect causes a hole in the septum separating the lower left and right chambers called the ventricles. As a result, the non-oxygenated blood in the right ventricles mix with the oxygen rich blood in the left ventricles.

3.       Right ventricular hypertrophy

This structural defect of the right ventricles of the heart produces thickened right ventricular muscles as a result of overloading the right ventricle in working harder in order to pump the blood through the narrowed right pulmonary valve (pulmonary stenosis).

4.       Overriding aorta

The aorta is the part of the heart that carries the oxygen rich blood and is responsible for pumping the blood from the heart throughout the body. In the presence of the three other structural defects of the heart, the aorta has to work more than normal, thereby resulting in extremely fatigued and overworked aorta.

Symptoms of Tetralogy of Fallot

The common symptoms of the disease consist of cyanosis, a bluish looking skin color because of the lack of oxygen circulating in the body. Dizziness, fainting, and irregular heartbeat are common symptoms of the disease. Some may also experience seizures and delayed in growth and development due to the lack of oxygen circulating in the body. Those with Tetralogy of Fallot are also at higher risk of endocarditis.

Diagnosis and treatment

Proper diagnosis of Tretatology of Fallot is taken during pregnancy or when the child is born. Screening tests such as a prenatal examination can help diagnose the presence of birth defects. Ultrasound test can also identify abnormal heart development of the heart during pregnancy. A positive findings of heart abnormalities are usually confirmed by a fetal echocardiogram, which monitors the fetal heartbeat. Babies that are born with Tetratoly of Fallot usually manifest an episode of turning blue when feeding or crying. Common telltale signs and symptoms of the disease is a bluish looking skin and a whooshing sound called a heart murmur upon examination. Surgical intervention is the treatment for Tetralogy of Fallot after childbirth. The defective heart structures are replaced or widened in order to improve the flow of blood and oxygen through the heart.

Sometimes the immune system identifies the body's own healthy cells as foreign, and attacks them; this phenomenon is known as autoimmunity or autoimmune disease.

Autoimmune diseases are chronic illnesses that can cause serious health problems. Most people with autoimmune disease need on-going medical care; yet many also encounter serious obstacles to receiving that care.

The obstacles that autoimmune patients encounter are many and varied and they occur in both the diagnostic and treatment stages.

Obstacles to Diagnosis

One of the major obstacles to diagnosis is that doctors don't always think to screen for autoimmune diseases. Patients with autoimmune disease often have to visit multiple doctors over several years before someone thinks to run an antibody test or conduct any other autoimmune diagnostic test.

Some patients persist until they find that doctor. Others give up, from fatigue or frustration, only to get a diagnosis when the diseased has progressed enough to do serious damage.

For example: A patient goes to her doctor complaining of hypothyroid symptoms. The doctor orders the standard thyroid panel — the TSH, T3 and T4 test “ which comes back normal. He tells the patient that there's nothing wrong yet the patient's symptoms worsen as time passes. It's not until the patient finds another doctor who decides to order an autoimmune thyroid panel that she learns that has Hashimoto's thyroiditis.

By the time that happens, she could have developed Hashimoto's complications like heart problems, myxedema, and mental health issues.

Obstacles to Treatment

The biggest obstacle to treatment is that autoimmune diseases have no cure and most of the treatments are focused on either managing the symptoms, or suppressing the immune system. Many of the treatments can also have serious side effects. For many patients, the treatment side effects can lead to compliance issues.

For example, a doctor could prescribe a lupus patient pain killers and steroids to treat inflammation, and immunosuppressive drugs to prevent the antibodies from attacking her organs. The pain killers could make her mental processes foggy, the steroids could cause fluid retention and mood swings, and the immunosuppressive drugs make her more prone to infections. As a result she could stop taking some or all of her meds.

Another obstacle is lack of patient access to the tools they need to manage their own care.

For example, Beckers Hospital Review reports that a large percentage of hospitals have inadequate websites with regards to patient care and services. Sixty-seven percent of the hospitals they reviewed did not have any sort of online patient rehabilitation or aftercare information; and almost 50 percent had no mobile patient websites. Other problem areas included pre-registration for medical procedures, bill pay, and prescription refill requests.

Patients with autoimmune diseases often make use of hospital and clinic services, especially if they need surgery or chemotherapy as part of their treatment. These patients also have serious issues with pain and fatigue, which makes even simple things like waiting in line, difficult. Not having access to a way to register for treatments, or order prescription refills from home or through a mobile app, often leads to compliance issues.

Access to effective health care is another major obstacle. Although the Affordable Care Act has made it easier for lower income individuals to have access to health insurance, there are still individuals who do not have coverage either because their states have not extended Medicaid. For those patients, the treatments are often too expensive, which often leads to compliance issues.

Red face and rosacea

Rosacea or red face is a skin problem that affects the face. It is also known as acne rosacea because of its similarity to acne. Generally the following symptoms of rosacea are seen-

    • Frequent flushing of the face. This is the first symptom of the problem. It may also be the only symptom for a long duration of time before any other complication develops.
    • Redness of the face. This redness occurs only in some parts of the face. It looks similar to sunburn.
    • Telangiectasia are seen on the face. These are tiny blood vessels which have found under the skin. They become prominent on the face in case of rosacea.

Rosacea treatments

General treatment

Facial flushing treatment

The feeling of panic and anxiety are natural responses of the body in the presence of dangers and under stressful circumstances. Most of the time, the level of anxiety differs from one person to another while a panic attack is usually manifested as a consequence of the feeling of anxiety. While this can be considered as a normal behavioral response, when they are manifest with extreme responses not in proportion to the current condition, panic and anxiety become a major psychological disturbance.

What is anxiety and panic attack?

An anxiety attack is a normal body response to stressful situations. It is in fact a natural response of the body when it senses danger. Anxiety always makes a person alert and focused and serves as a motivating factor to take action. It only becomes a problem once the anxiety attack symptoms become quite overwhelming and persistent resulting in the interference of the person's ability to perform normal functions and affect relationships with others. Awareness of the common anxiety attack symptoms is a helpful indicator to gauge whether your anxiety is one with minor or major severity that requires proper diagnosis and treatment.

Panic disorder mainly takes a form of an anxiety which is characterized by a sudden panic attack that may be recurrent, resulting in behavioral changes that may last at least a month. This condition can become an anticipatory disorder where the person has an ongoing worry and predicaments of having another attack. Panic disorder is a treatable condition even without any known cure. The condition typically occurs during adulthood between the ages of 25 to 30 years old affecting women twice as much as men. Some sources would say the condition may occur as early as 15 to 19 years of age.

When panic and anxiety occur

One striking characteristic of panic attacks experienced by a person is that the attack is very sudden and unexpected. It occurs unprovoked and can often become disabling. The person with panic disorder may often develop phobias or irrational fear of situations that preceded the panic attack and begin to avoid them. This pattern of avoidance then eventually becomes a different level of anxiety where the mere idea of doing the things that preceded their first panic attack usually triggers a future panic attack with a resulting condition of agoraphobia. Panic attacks may continue for months or even years, which duration is highly dependable on when and how treatment is sought. If left untreated the extent of anxiety and panic attacks can become distressing to the person to the point that they make an effort of concealing their condition because of the fear of the stigma of mental illness.

Individuals with severe and repeated panic attacks can become disabled by the condition. Extreme fear of experiencing the attack again, which is often termed agoraphobia usually leads to phobias and the individual no longer enjoys doing their usual activities in the fear that they will have another panic attack. Hence the person's activities become restricted.

Symptoms of panic and anxiety disorder

There are innumerable signs of anxiety attack that may involve emotional, psychological and physiological reactions. Fear and anxiousness are the most common signs of an anxiety attack. Early and minor anxiety attack symptoms may range from the feelings of apprehension, restlessness, fear, irritability, and tension. One may also experience physiological response of the body, such as pounding heart, sweating, muscle tension, frequent urination, diarrhea, insomnia and fatigue.

These signs of anxiety are common in minor panic attacks but there are major anxiety attack symptoms that are considered disproportionate responses than a particular situation warrants. This result to uncontrolled anxiety, restlessness, depression and extreme fatigue. The anxiety attack symptoms become persistent that it affects the person's ability to cope with stress that impairs their activities of daily living.

It is also possible that panic attacks may be continuous or may occur in waves of one panic attack after another within an hour period. However, one should note that if the symptoms of attack persist for more than an hour it is likely you might not have a panic attack disorder and should immediately seek medical treatment.

The following are the common symptoms seen in panic disorder:

  • Pounding heart
  • Chest pain
  • Difficulty in breathing or dyspnea
  • Intense fear and terror
  • Dizziness or fainting
  • Sweating
  • Choking sensation
  • Trembling/shaking
  • Chills or hot flushes
  • Numbness or tingling in the finger and toes
  • Nausea
  • Stomachache
  • The fear of losing control or about to die
  • Rapid heartbeat
  • Feeling of being detached from reality
  • Persistent fear of having another attack

There are also specific symptoms that are highly suggestive of Panic Disorder which should be at least one of the attacks followed by a month or more of one or more of the following symptoms:

  • Persistent concern about having additional attacks
  • Significant change in behavior related to the attack
  • Worry about the implications of the attack and its consequence, such as having a heart attack, going crazy and losing control

  • Substance abuse and alcohol

  • Dysfunctional part of the brain called amygdala can cause unnecessary stimulation of the body's fear response.
  • Eating disorders such as anorexia and bulimia nervosa
  • Stimulants such as caffeine and nicotine
  • Chemicals such as carbon monoxide in tobacco smoke

 Treatment for panic and anxiety

Treatment for the disorder usually involves drug intervention and cognitive therapy, relaxation techniques and other forms like psychodynamic and the like.

Medications

Commonly prescribed medication are antidepressants and anti-anxiety drugs (benzodiazepines). Antidepressants can reduce the panic severity and help eliminate panic attacks. Selective Serotonin Reuptake Inhibitors (SSRIs) and Tricyclic antidepressants are found to be effective in reducing the severity of the attacks. Anti-anxiety drugs helps to prevent the occurrence of panic attacks but it may become a habit forming practice.

Cognitive behavioral therapy is found to be the most effective treatment for panic and anxiety disorder. Heel spur (Calcaneal spur) is a bonny outgrowth of the heel bone which is pointy in shape. It's a calcium deposit that happens under the heel bone.
