Insomnia

Can't sleep? Or do you often struggle to get to sleep no matter how tired you are? Or do you sometimes lose sleep in the middle of the night and lie awake staring at the clock anxiously?  You aren't alone, a shocking 50-70 million Americans and more in the world have sleep difficulties. Insomnia is a recurrent problem that can deprive you of energy, change your mood and even affect your general health and ability to carry out daily activities effectively. Sometimes, chronic insomnia can manifest in serious health problems. But contrary to common believe, insomnia isn't a sleep disorder. Insomnia is to a larger extent a sleep complaint. With simple lifestyle changes and change of daily habits you can put a stop to sleepless nights.

If you are spending hours staring at the clock at night or know someone suffering from insomnia, here are tips that can help correct the sleep complaint.

Put a check on your caffeine intake

Check the quantity of caffeine you are consuming each day, especially in the evenings. It is advisable to assess your diet and identify the sources of your caffeine: maybe it's the soda you take over lunch, random energy drink or a cup of coffee and/or medications.  Large quantities of caffeine in the body are recipe for body jitters and can cause restlessness. Also, the additional sugar in sodas and energy drinks as well as that added in coffee can culminate in sleep issues.

Reduce Stress

It's much easier said than done but completely reducing or at least managing your stress in a sustainable manner will contribute positively towards overcoming your insomnia. Stress is known to cause mental instability, especially when you stay awake pondering over countless things. Try to solve all issues that may make deprive you of your sleep before retiring to bed. If you had any tasks like laundry or any other house chores or work related projects, ensure you complete it before going to bed. In doing this not only are you meeting the goals of your to-do list, but also shedding off stress that will negatively impact on your schedule tomorrow.

Obviously, it is not possible to cut stress completely; all we can do is identify ways to effectively manage the stress levels. Be willing to embrace help when offered, be liberal to designate some tasks to other people and/or research around for different stress management books that are readily available.

Know your medications

In case you are on medication, take a minute or so to read through the side effects that are mostly disclosed in the included sheet. If you happen to have any concerns, feel free to seek clarification from your pharmacist or doctor.  Some medications have been found to contain caffeine especially: antidepressants; or any other medicines used to treat cold and flu as they have alcohol traces; pain relievers; diuretics; corticosteroids; thyroid hormones and high blood pressure prescription medications.

Medical Conditions

Some medical conditions manifest in sleeplessness and cause insomnia. The most common medical conditions associated with insomnia include: asthma, allergies, Parkinson's disease, hyperthyroidism, acid reflux, kidney disease, cancer or chronic pain. Incase you are suffering from any of these conditions or other isolated health ailments that cause insomnia its time to make an appointment with your physician.

Your physician may recommend some medications and/or physical therapy that you can incorporate into your life and can significantly ease the underlying issues so as to get some much-needed sleep.

Insomnia

Consider Your Sleep Environment

Put into consideration your sleep environment and ensure the surroundings are conducive for sleep. How is the lighting?  Is the area overheated? Do you work from your laptop or desktop in the bedroom?  Ask yourself all these important questions and try to resolve the issues that cause insomnia.  Perhaps it is time you considered adjusting the lighting, purchased accommodating pillows, and adjust the thermostat as well as desist from bringing your work into the sleep area.

Make your sleeping environment work for you and filter what is keeping you up at night in addition to your lifestyle.

Sadness and stress are the road to depression. This psychological condition affects millions of people, and the number of antidepressant prescriptions proves it. The worst thing about these medications is that they are drugs, and can be as addictive and harmful as amphetamines or LSD.

Improve Vision

Over four out of five people are of the idea that sight is undoubtedly the most useful sense the have. More important than the other four senses. All this can be attributed to the consideration that almost 80 percent of all human perceptions originate from our sight.

On a daily basis, good eyesight is very integral in aiding your movement and enjoyment of all aspects that make life more enjoyable. So it undoubtedly make sense if you used a vision improving method to enhance eyesight as to the highest limits, without necessarily using artificial aids that are likely to make your vision worse with continuous use.

Why you need a vision improvement method

In as much as many people believe that eyeglasses enhance other people's vision, and to a lesser extent eliminate painful episodes and discomfort, in the long-term they result sight defects, and sometimes never enhance your vision.

Vision Without Glasses™

Rarely do people realize that most recurrent sight abnormalities are rooted on a dynamic set of processes that are varied in nature and regularly vary during the course of the day based on various factors.  They are simply not set on stone. Any physician will confirm to you that most children will have good eyesight in the early morning, but imperfections manifest in the early afternoon, or vice versa. This applies to both children and adults alike.

The extent of visual impairment varies greatly over extended spheres. The duration is also different. Under certain conditions the imperfection can persist for very quite sometime, while in other conditions it might hinder the subject from having a clear look of things normally, for days, weeks, or months or even years.

The facts remains that we all lack a consistent and consistent visual impairment. It varies greatly in accordance to the light conditions across the day.

Eyeglasses DON'T compensate for imperfections in variable vision.

In today's “spinning door” era of controlled healthcare, it is no longer surprising that a majority of doctors will conveniently prescribe eyeglasses or even suggest a surgery without budging. And when we face it critically “ that is part of their training as pertains to treatment of vision they're not about to reverse the tactic overnight.

Glasses only treat the symptoms NOT the problem and radical damage to your eyes goes unchecked.

So if a doctor tells you there's nothing they can do today, and there isn't another way to enhance your vision, you need to bear in mind these facts. Prescription of stronger glasses today will not address your underlying cause of poor vision health, it only means that you will only need stronger glasses on your next visit a few months down the line.

How to restore your vision naturally

Improve your visionWhen you are stressed it is likely that your eye usually goes partially or completely blind. In some cases, your sight goes blind first, for short or longer periods, which is mostly guided by the extent of strain; if the impact of the strain is strong enough then a significant part of your retina may be blinded.

A vision improving method will ensure that your eye aligns to a central fixation; not only for perfect vision, but also for perfect relaxation and rest meaning it can be utilized unconditionally without fatigue. This means that, a vision improving method doesn't cause any muscular insufficiencies.

The muscles on your face and the entire body are always relaxed, and if the condition is repeated you won't have wrinkles or dark rings lining your eyes.

A vision improving method simply focuses on obtaining relaxation, to ensure that all visual imperfections are corrected from the root cause, with focus on ensuring that you can read and see clearly without straining you eye muscles to as you try to enhance your vision.

SweatyPalm

Do you find it embarrassing when you shake hands with other people only to find out your palm is sweating profusely? This is not an isolated case as many experience this kind of problem when it comes to clammy hands. The moisture your hands leave after touching something or your desk showing traces of your sweaty palms shows how embarrassing and disgusting it is.

Clammy hands are different from the regular sweat our body produces while doing strenuous activity. What makes up most of our sweat glands is what you call the eccline glands. This gland is said to be specifically concentrated on the palms, forehead and the feet. When they are stimulated, they release an excessive fluid that should control the body temperature in which in other cases the reason behind your sweaty palms. People who have dealt with  sweaty palms for the longest time have learned to adjust to it, thus ignoring the nuisance it sometimes cause.

How to get rid of sweaty palms:

  • Regularly soaking your hand in water with used tea bags will help reduce the sweat in your palms. The tannic acid that is present in teas will help dry out your palms. Doing this everyday for more than a week will help reduce the risk of getting sweaty palms.
  • Apply antiperspirant in your palm. This is not necessarily a deodorant. You can apply powder or lotion with antiperspirant capability. Make sure they are not scented to leave the hand, looking and smelling normal.
  • Drysol is an antiperspirant agent but can only be acquired through a doctor's prescription. This type of antiperspirant is known to have a side effect that is why it is very important to consult your doctor before proceeding.
  • Believe it or not, sweaty palms are also caused by hormonal changes as well as emotional stress. If you think you suffer from these types of changes, you might need to see a therapist in order to help you adjust and maintain a balance disposition.
  • Wash your hands regularly. For you to able to avoid the back of your hand to get dry and scaly, use regular soap to wash your palms by rubbing them together and wash the back of your hands with plain water.
  • You can also bring with you a hand sanitizer, the one that doesn't contain excessive moisturizer.
  • Always bring with you a handkerchief or a tissue paper that you can use to wipe off the sweat on your palms.
  • Sprinkling baby powder or baking soda will help dry your palms.
  • Foot powder is another solution for a sweaty palm. Sprinkle just a little of it on your palms and you'll that it works better than a regular powder in absorbing sweat from your hands.

Treatments options for sweaty palms

When the sympathetic nervous system is very active and stimulates the sweat glands too much, they will produce more sweat than is needed to regulate the body’s temperature. While no one is sure why the sympathetic nervous system sometimes acts this way, different treatments can calm down all the activity. Aside from the home remedies mentioned above, there are other solutions available to help you get rid of sweaty palms. Some people lose hope about getting permanent treatment for their sweaty palms. Regardless of the condition, there have been numerous home remedies that are proven to be effective and they don't cost much for you to do at home.

A sage tea treatment,  for instance, is one of the cheapest ways you can reduce the occurrence of sweaty palms by dipping your hands in a sage tea solution for 30 minutes. By religiously doing this every night before sleeping, you will notice the big difference it will make to your sweaty palms. You can also opt to drink sage tea regularly. Past studies show that tomatoes can help cool down the body temperature. By drinking at least two glasses a day, you are aiding your body to a lower temperature causing your palms to stay dry and comfortable. The natural drying solution of the apple cider vinegar mixed with water reduces the sweat on the palms remains to be a popular home remedy for a sweaty palm.

Excessive sweating on the palm region of your hands is called Palmar hyperhidrosis.  People suffering from this kind of burden usually find this as interference in their daily activities. The most common problem they relate to clammy hands is the embarrassment when they shake hands with other people. The cause of sweaty palms can be categorized into two. It can be caused by intense emotional stress and your body adjusting to its temperature or what you call thermoregulation.

Prescription antiperspirants are usually the initial medication applied to palmar hyperhidrosis. This type of medication is a solution that is rubbed into the palms to prevent it from sweating. A botulinum injection on the other hand is the process of obstructing signal to the hand's nerves thus preventing it from producing sweat. Iontophoresis is the process of stimulating electrical shocks which numbs the sweat glands that disallows the formation of sweat on the palms.

The aforementioned treatments for sweaty palms are indicated based on studies and actual medical treatment performed by doctors. Not every person suffering from clammy hands is subject to said treatments as they may vary on the medical requirement.

Anxiety is a feeling of constant fear that often has no reasonable grounds for its occurrence. It is characterized by a considerable number of psychological symptoms such as stress, irritability, and sweating. It serves as a motivation to deal with different problems, but abnormal functioning of it leads to a serious disorder that manifests in almost one third of the Earth's population.

Individuals with chronic obstructive pulmonary disease are at high risk for the development of acute respiratory failure. Altered host defenses, increased secretions volume and viscosity, impaired secretion clearance and airway changes and common pathophysiologic changes predispose patients with COPD to frequent episodes of ARF. The etiology, clinical presentation and management of COPD patients vary somewhat from ARF without chronic underlying pulmonary dysfunction.

COPD

The anatomic changes in COPD

Etiology, Risk Factors, and Pathophysiology

Any systemic or pulmonary illness can precipitate ARF in patients with COPD. Precipitating events of acute respiratory failure in COPD include decreased ventilator drive, decreased muscle strength such as with malnutrition and shock, decreased chest wall elasticity such as in patients rib fractures and pleural effusion, decreased lung capacity for gas exchange, increased airway resistance, and increased metabolic oxygen requirements. In addition to the etiologies of ARF, diseases as well as situations that decrease ventilator drive, muscle strength, chest wall elasticity, or gas exchange capacity, or increase airway resistance or metabolic oxygen requirements can easily lead to ARF in patients with COPD.  The most common precipitating events include:

  1. Airway infection. Frequent antibiotic administration, hospitalization, and impaired cough and host defenses in COPD increase acute airway infections. Infections are commonly caused by gram-negative enteric bacteria or Legionella, with Haemophilus influenza and Streptococcus pneumonia causing bronchitis.
  2. Pulmonary embolus. The high incidence risk of right ventricular failure in COPD increases the risk of pulmonary embolus from right ventricular mural thrombi.
  3. Heart failure. In the presence of pulmonary hypertension and right sided heart failure, treatment of left-sided heart failure is often delayed because of difficulties in early diagnosis.
  4. Noncompliance with medication regime. The complicated treatment regimen in the management of COPD which includes frequent administration of both oral and inhaled agents, frequently leads to the under use of medications.

The development of ARF in patients with COPD places a tremendous burden on the pulmonary system. The chronic disease process leads to the impairment of ventilation, poor gas exchange, and airway obstruction. The additional burden of acute disease process, even relatively a minor one, further impairs ventilation and gas exchange and increases airway obstruction.

The signs and symptoms are ARF includes:

  • Hypoxemia (PaO2 <60 mmHg)
  • Restlessness
  • Tachypneas
  • Dyspnea
  • Tachycardia
  • Confusion
  • Diaphoresis
  • Anxiety
  • Hypercabia (PaCO2 >50 mmHg)
  • Hypertension
  • Irritability
  • Somnolence (late sign)
  • Cyanosis (late sign)
  • Loss of consciousness  (late sign)
  • Pallor or cyanosis of the skin
  • Use of accessory muscles of respiration
  • Abnormal breath sounds

Principles of Management for ARF in Patient with COPD

Treat the Underlying Disease state

Treatment is directed at both the acute precipitating event and the chronic airway obstruction problems associated with COPD.

  1. Bronchodilators and corticosteroids are usually used to increase the airway diameter and reduce airway edema respectively. Higher than usual doses may be necessary until the precipitating event is resolved. However, brochospasm that is refractory to bronchodilators in severe asthma cases may require subcutaneous epinephrine administration.
  2. Antibiotics to treat pulmonary infections
  3. Improve secretion removal by adequate hydration, corticosteroids, coughing, heated moist aerosolization and chest physiotherapy.

Improving oxygenation and ventilation

  1. Correction of hypoxemia by small increases in FiO2 levels, preferably with a controlled oxygen delivery with devices such as a Venturi mask, biphasic intermittent positive airway (BIPAP). Arterial blood gases are frequently monitored to ensure adequate arterial oxygenation. Higher than necessary FiO2 should be avoided since it may suppress the hypoxic ventilator drive of the COPD patient. Titration of oxygen administration should be guided by pH and PaCO2.
  2. Positioning of the patient is also necessary in maximizing ventilatory efforts and relaxation during spontaneous breathing. High Fowler position, leaning on an overbed table is often the greatest position of comfort for COPD patients prior to intubation.
  3. Relaxation techniques and pursed lip breathing may be helpful to decrease anxiety and improve ventilatory patterns. Anxiolytics and other sedatives should be used cautiously.
  4. Intubation and mechanical ventilation are based primarily on the deterioration of mental status coupled with knowledge of the patient's baseline pulmonary function and functional status and the reversibility of the underlying cause. Somnolence and lack of cooperation with treatments are strong indications for intubation. Weaning from the mechanical ventilation is frequently more difficult, and in some cases impossible for patients with COPD. Informed discussions with the patient and the family regarding intubation should be undertaken.
COPD

Fowler position is most ideal for COPD

Slow correction of hypercarbia should be done to avoid life threatening alkalemia from preexisting metabolic compensation. The development of auto PEEP and barotrauma is increased in patients with COPD, necessitating smaller tidal volumes, higher respiratory rates and short inspiratory and long expiratory times.

Nutritional support

Typically, patients with COPD have protein-calorie malnutrition as well as low levels of electrolytes such as phosphate, magnesium and calcium. These chronic nutritional deficits lead to muscle weakness and may interfere with the weaning process. Early enteral or parenteral feeding of these patients is essential to avoid further deterioration in their nutritional status during acute illness. Parenteral feeding may be best initially since dyspnea in non-intubated patient makes oral feeding more difficult. Lipid calories should also account for 50% of the nutritional support during mechanical ventilation.

Maintaining the body's ideal weight is likewise important as part of the nutritional goals of a patient with COPD. Good nutrition will help the body become strong against infections. Eating a healthy diet is essential for proper body weight maintenance and seeing a dietician will help you define the best nutritional plan with high consideration to your condition. Being overweight can induce more stress to both the lungs and heart which can exacerbate COPD and might increase the risk to an acute respiratory failure. On the other hand, being underweight can make the body weak and more susceptible to other diseases and infection. To keep the body strong against COPD complications, the combination of regular exercise and a healthy diet will provide the body sufficient energy and strength.

Drinking fluids of up to 6 to 8 ounces of water or non-caffeinated drinks will have a good effect in thinning the mucus which makes coughing easier and less burdensome. It is also best to avoid certain beverages that might interact with your medications like tea, caffeinated drinks, cola and other carbonated drinks. Fiber rich diet is also ideal in helping the digestion process that helps control the body glucose levels. At least 20 to 35 grams of fiber consumption each day will be adequate in supplying the fiber needs of the body.

Preventing and managing complications

Healthcare team should also monitor complications commonly observed in COPD patients:

  1. Arrhythmia: high incidence of both atrial and ventricular arrhythmia in patients with COPD because of hypoxemia, acidosis, heart disease, medications and electrolyte abnormalities. Cardiac monitoring and treatment of the underlying cause is the focus and antiarrhythmic medications should be given only for life threatening situations.
  2. Pulmonary embolus: observe for signs and symptoms and follow the usual treatment and prevention guidelines as this has high incidence risk
  3. GI distention and ileus: very common in dyspneic patients, increasing incidence of this complication
  4. Auto-PEEP and barotrauma: high incidence especially in elderly and patients with high ventilation needs.

An acute respiratory failure can become an emergency situation. In order to ensure that you are in a healthy condition, make sure to see your doctor regularly.
